For your convenience, we have listed helpful information, along with some
useful forms, that will assist in the transfer of your accounts from other
financial institutions to an account here at The Condon National Bank. These
forms must be COMPLETED AND PRINTED. They CANNOT be submitted online. If you

Step 1. Getting Started. Complete the Information Guideline to help gather
information about your new and existing accounts, direct deposits and automatic
payments.
Step 2. Open New Accounts. Visit our Main Bank location at 814 Walnut,
Coffeyville, to open your new account with The Condon National Bank. Please
complete the New Account Form, and bring with you when you open your account.
This will expedite the account opening process. You will also need to bring your
drivers license or state identification card, and another form of
identification.
Step 3. Change Automatic Transactions. Click on the Automatic Payment/Deposit
Switch Form or Payroll Direct Deposit Authorization, which will assist you in
contacting companies that handle your automatic deposits and withdrawals. If you
have social security or other federal payments, go to www.ssa.gov/deposit/howtosign.htm
to complete and print the Direct Deposit form. Your monthly automatic payments
may include: electricity, water, gas, telephone, cable, gym membership, internet
provider fees, loan payments, etc. You will also need to contact your pay source
for automatic deposits, such as your employer, retirement funds provider or
brokerage firm. If you have any questions, or would like assistance with
completing these forms, we will gladly assist you.
Step 4. Close Old Accounts
The initial direct deposit or automatic payments may not occur immediately, so
please verify that your direct deposit and automatic payments have been changed
at the source and then received through your new account with us before closing
your existing account.
We have attached an Account Balance Worksheet. This will assist you in
determining how much money to leave in your existing account. When all items
have cleared, complete the Existing Account Closing Form, and mail to your
previous Financial Institution. If you have checks and deposit tickets from your
previous Financial Institution, either destroy them, or bring them in and we’ll
shred them for you. Do not throw them in the trash! Be sure to destroy any old
ATM or CheckCards. Print off and check the new Account Conversion Checklist to
verify the steps you have taken in the process.
Step 5. Transfer other accounts
You may have Individual Retirement Accounts, 401(k) funds, certificates of
deposit or loans with other sources. We will assist you in transferring these
other accounts. Please see a Customer Service Representative, and we will
coordinate a plan to consolidate your accounts here at The Condon National Bank.

Important Information about Procedures for Opening a New Account
To help the government fight the funding of terrorism and money laundering
activities, Federal law requires all financial institutions to obtain, verify
and record information that identifies each person who opens or is added to an
account.
What this means for you: When you open or add signers to an account, we will ask
for the name, address, date of birth and other information that will allow us to
identify all signers. We may also ask to see driver’s licenses or other
identifying documents. We cannot make any exceptions.
